    No date...appears to be from the 1940s or early 1950s era , as phone # is 4-9211.
    light spotting/discolor to front stiff cover.    back cover has scoring instructions.

    there are four score sheet attached pages , all used / damaged , as a couple have some insect damage to top/right corner of page.   The inside pages have writing to the pages.
